Crash while merging on a highway
These accidents happen when one vehicle is merging and it collides with a vehicle that is approaching on the highway.
In this diagram, Vehicle A is crossing a broken line and leaving a yield sign to merge onto the highway when it collides with Vehicle B, which is approaching on the highway.

The rules of the road
Drivers must not drive from one lane to another unless it is safe to do so and must yield to the right of way to all other traffic if there is a yield sign. Therefore, the driver of Vehicle A would be found 100% responsible.
