1. Fill out the application form
Fill out the electronic funds transfer (direct deposit) authorization and direction form
2. Make sure your information is correct and complete
Before submitting your application form, please make sure that:
Your form is signed
Your information is complete and written clearly
The bank account provided is in your own name
The bank account provided is held by an adult
3. Email us the form and a void cheque
Send the form and a void cheque or completed direct deposit form from your bank by email to [email protected]
Email applications are typically processed within 30 days. To receive claim payments sooner, sign up for direct deposit online.
